Cloudline Physical Therapy LLC (“Cloudline”) is committed to protecting the privacy and security of our patients’ personal and health information. This Notice describes how we may use and disclose protected health information (“PHI”), your rights regarding that information, and our responsibilities under applicable federal and state privacy laws. PHI includes information that identifies you and relates to your health, health care, or payment for health care services. We are required by law to maintain the privacy and security of your PHI and to follow the terms of this Notice.

Uses and Disclosures of Protected Health Information

Cloudline may use and disclose your PHI for treatment, payment, and health care operations. This may include coordinating your care with physicians and other health care providers, communicating with insurance companies regarding eligibility, authorization, claims, and payment, and conducting activities necessary to operate our practice, including billing, quality improvement, staff training, compliance, and administration.

We may share PHI with third-party service providers that perform services on our behalf, such as billing, electronic health record, technology, or administrative services. When required by law, these organizations are contractually obligated to appropriately safeguard your information.

Unless you object, we may disclose relevant information to a family member, caregiver, or other person involved in your care or payment for your care. We may also use or disclose PHI when permitted or required by law, including for emergencies, public health and safety activities, workers’ compensation, health oversight, law enforcement, judicial or administrative proceedings, or other lawful purposes.

Certain uses or disclosures not otherwise permitted by law require your written authorization. You may revoke authorization in writing at any time, except to the extent that we have already acted upon it.

Communications and Mobile Information

Cloudline may use the contact information you provide to communicate with you regarding appointments, scheduling, treatment, billing, or other appropriate matters. No mobile information will be shared with third parties or affiliates for their own marketing or promotional purposes.

Your Privacy Rights

You have the right, subject to applicable law, to inspect or obtain copies of your health information, request corrections or amendments, request restrictions on certain uses or disclosures, request confidential communications, receive an accounting of certain disclosures, and obtain a paper copy of this Notice. You may also authorize another legally permitted individual to exercise these rights on your behalf.

Copies of Medical Records

You may request copies of your medical records in accordance with applicable federal and Pennsylvania law. For 2026, Pennsylvania permits fees of up to $2.00 per page for pages 1–20, $1.48 per page for pages 21–60, and $0.52 per page for pages 61 and thereafter, plus actual postage, shipping, and delivery costs when applicable. A $29.61 search and retrieval fee may be charged where permitted, but may not be charged when an individual requests their own personal health record. When a patient or their personal representative requests the patient's own records, fees will be limited to the reasonable, cost-based amounts permitted by HIPAA. Cloudline may charge $15.00 per hour for labor directly associated with copying or preparing the requested records, to the extent permitted by law, in addition to allowable costs for supplies and postage. Search and retrieval costs will not be charged for these requests.

Our Responsibilities

Cloudline is required to maintain the privacy and security of your PHI, provide this Notice, and notify affected individuals following a breach of unsecured PHI when required by law. We reserve the right to modify this Notice and our privacy practices as permitted by law. Any revised Notice may apply to information we already maintain as well as information received in the future, and the current version will be available on our website.

Complaints

If you believe your privacy rights have been violated, you may contact Cloudline Physical Therapy at info@cloudlinept.com. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Cloudline will not retaliate against you for filing a complaint.
